Caseworkers and DFPS Staff:


Please be sure that you have confirmed receipt of 2054 to therapist.


You and your client will be permitted portal access to make, change, and address appointments.

This is specifically useful should you need to schedule a 1:1 meeting to discuss a case that requires an in-depth discussion.


Case Management meetings with therapists are preferential on Fridays.


Your portal access will permit your to schedule appointments for your client; caseworkers will not have full access to client notes and personal data.


Clients and caseworkers MUST UPLOAD an ID to the portal.


Caseworkers MUST send written authorization for telehealth services.


All Conservatorship Cases MUST include: All pertinent case documents (to include psych evaluations and past counseling data), All attorneys on file, Caseworker contact information, Caseworker supervisor information, 2054 for authorized services (with specific written telehealth permission), upcoming court dates, and regularly scheduled case conceptualization meetings.


**If accurate attorney and caseworker information is not provided through the portal, you may hinder your case through lack of communication. If you do not provide court hearing data, your court documents may not be prepared in a timeframe that is needed for case success.


Clients and Caseworkers may apply for intake and upload case documentation directly to the portal; follow-up with secretary and/or owner to insure a successful intake application (254-258-2756   254-371-4661).


Clients: Homework will be uploaded to the portal for your access. Your homework answers may be uploaded to the portal to expedite communication of your self-paced progress. If you are struggling to access the portal, please click on “client portal” on this page and follow the prompts for access.


**Do not call the office requesting help with an appointment or homework until you have accessed your portal; Screenshot any issues with portal access and provide those during any requests via a phone call.


IMPORTANT TO KNOW:


  • If clients miss 2 or more appointments they will be unsuccessfully discharged
  • If you are more than 15 minutes late you may lose your appointments
  • You will have notes sent to your caseworker approximately every month
  • You will have an opportunity for case review and changes every 90 days
  • You ARE required to sign the Release of Information to all parties associated with your case
  • If therapist is subpoenaed to court outside of DFPS contract, subpoenaing party is subject to therapists hourly fee and a $1000 court appearance fee
  • Haven of Hope therapists ARE NOT EMPLOYED by DFPS
  • All therapists reserve the right to refuse a case based on perception of liability or inability to tend to case effectively which includes uncooperative or non-communicative staff AND clients
